What to expect on your first visit.

First Visit

On the first visit to North Florida Chiropractic Physical Therapy in Orange Park you will be greeted by the receptionist and given a packet to fill out. The packet includes information regarding your case history and chief complaint. The paper work generally takes 15 to 25 minutes to complete; and it is very important that all paper work is completed because it encompasses your medical record. At the time that you return your paper work please provide your drivers license and your insurance card at which time the receptionist will give you a brief overview of your insurance coverage; by your second visit we will have verified your coverage and determined your eligibility and benefits including co-pay and any out of pocket expense.

Next, you will be seen by the doctor for a physical examination including range of motion analysis, postural analysis and orthopedic screening also the doctor will ask you questions regarding your injury and how it happened; at no time you will be asked to disrobe. After the examination is completed, the doctor will decide whether or not to take X-rays or other imaging studies; if X-rays are deemed necessary we have a complete X-ray facility on site. The doctor will then go over a report of findings including a working diagnosis with you generally on the same day or your next visit.

The first visit is critical as the doctor will discuss with your treatment options specific to your case, at which time you as a patient will choose whether or not to receive treatment, if you decide to be treated the treatment will be performed at that time. After the treatment is completed the doctor will escort you to the front reception area to recommend a conservative treatment schedule for you. A treatment schedule may very from one time a week to 3 times per week for a given period of time depending on the severity of the problem or injury. We understand a treatment schedule may be difficult; this is why we are open evenings and weekends for your convenience.

During the first visit your treatment schedule will be written down for you by the receptionist on an index card. The receptionist will give you friendly reminders calls throughout the week. If you choose the receptionist can place you in a cancellation list in case we have last minute cancellation. It is vital to your healing process to keep your appointments. Once you have an appointment scheduled if you cannot make it for any reason, please notify the receptionist within 24 hours of your scheduled time to avoid a cancellation fee. Payment for treatment is due at the time services are rendered unless other arrangements have been made.
